By checking the boxes below, I am attesting I have read the Library's mission statement and agree to abide by the Library's Code of Conduct and the following:

All presenters, performers, and programs are subject to approval.
Approval of program(s) does not guarantee a booking.
Scheduling programs is at the discretion of each branch library. All inquiries about booking must only be conducted through the Learning Experiences Unit. Presenters should not contact library branches directly to inquire about bookings once approved.
The library will be responsible for all promotion and publicity for these programs, which will be free of charge and open to the public or by special invitation to target audiences.
I understand I may be subject to a background check, and that this background check may periodically require renewal.
If I have employees or other individuals who will present a program on my behalf, I understand that I must coordinate their background check with the Learning Experiences unit at least 14 business days prior to the event.
I understand that I will have 48 hours to complete the background check requirements per instructions to be sent by Hillsborough County. (Sent by Total Insight Screening to email address provided on the application).
I understand that I must provide at least three references with contact information.
I understand that I must provide a valid W9 form in order to receive any payment.
The program fee listed is inclusive of time, travel, supplies, and all other charges. No additional fees may be charged.
All supplies needed for attendees to participate in the program will be provided by the presenter.
Event spaces must be left in the same condition and arrangement in which they were found prior to usage.
Presenters agree to arrive at least 30 minutes before the program start time to allow for set up and check in with branch staff.
Virtual program or webinar presenters agree to schedule at least one tech practice session with library staff at least one week prior to the event.
In the event of an unexpected late arrival or cancellation, presenters agree to provide as much notice as possible via the notification methods listed on their offer letter.
A presenter may be considered a No Show if they have not arrived at least 15 minutes before a scheduled event, and may be cancelled.
The library reserves the right to cancel events at any time, especially due to unexpected operational concerns and closures such as hurricanes.
The library reserves the right to cancel the agreement and/or book an alternate presenter if the agreement is not confirmed and required paperwork is not received within 2 weeks of offer email.
Any information must be of general interest to the intended audience and applicable to situations that do not require purchasing a specific product or enrolling in a specific fee-based program.
Library programs may not be used to promote businesses, products, or for-profit enterprises.
Presenters are not allowed to require signups or collect contact information from program attendees. Presenters may provide contact information so that attendees can voluntarily contact them after the program has concluded.
Libraries are public spaces and pictures/video may be taken in the space by staff or members of the public.
Virtual programs may be recorded and/or used on the library's social media or other web platforms.
Accidents are to be immediately reported to library staff.
